Augmentation of an Extensive Anterior Defect

In this surgery Professor Dr. Hannes Wachtel, Munich/Germany, performs an augmentation in the anterior region using allogenic bone of porcine origin covered by membranes in the double layer technique.

Prof. Wachtel starts with raising a full flap using a sulcular incision and a micro scalpel blade. After opening up the defect an incision is performed to split the periosteum to increase flap mobility, which is necessary to ensure tension free wound closure and primary healing. Following using a pin bur perforating the compact bone layer to provoke bleeding a membrane of porcine origin (Evolution Membrane, Osteobiol) is placed firstly (outer membrane) and an additional membrane is added facing the residual bone (Ossix Membrane, inner membrane, “double layer technique”). Allogenic bone is placed in between the residual bone and the membrane and the defect is closed meticulously under the avoidance of any tension.
