Following the extraction of the teeth that were not worth retaining (12-16), this surgical procedure continues with immediate implantation and restoration using a fixed temporary bridge. Following the marginal incision and preparation of a mucoperiosteal flap, extraction is performed together with careful soft-tissue degranulation and smoothing of the existing bone edges. As is typical of conventional external sinus augmentation, the maxillary sinus is opened while taking care not to damage the membrane.
Once drilling has been performed several times, immediate implantation is performed in regions 15 and 12. The extraction sockets and exposed sinus are then filled with resorbable bone substitute. A membrane protects the augmentation area, preventing soft tissue from growing into it. Mobilising the flap using a periosteal incision and double-layer suturing comprised of non-resorbable material allows primary wound closure to be achieved.
Finally, following completion of immediate implantation, a screw-retained temporary bridge is integrated as an immediate restoration. Photos and x-ray images demonstrate a positive result that is stable in the long-term.
