Modified Rolled Flap Technique in the Posterior Mandible

The surgeon in this video, Professor Hannes Wachtel (Munich), uses a modified roll flap technique on the lower jaw in order to achieve thickening of the buccal soft tissue when exposing the implant in the posterior area. The individual steps of this surgical procedure are explained in detail.

To begin with, two incisions are performed up to the mucogingival junction. Following preparation of a split flap and the creation of a buccal pocket, the previously inserted implant is exposed using bone profilers and chisels, the cap screw removed, and a healing cap placed in position. The prepared split flap is folded into the buccal tunnel, thus facilitating the required thickening of the buccal tissue.

To ensure that the flap heals well, the epithelium of the split flap was first removed with a coarse diamond burr, until isolated bleeding points were visible. In the roll flap technique, a special suturing technique is used to secure the folded flap with microsurgical suture.
